This address is the automated callback Facebook uses to ask us to delete the data we hold for a Facebook account. It is not a page you need to fill in.
To remove the connection yourself, open your Facebook settings and remove EnAffaire from the apps connected to your account. Facebook then calls this address and we delete what we hold. You can also write to us and we will do it directly.
